Community-based Posts to gain special skills in certain Specialties

We also offer several additional posts , which count at SHO posts for the purposes of your three year training, but are community-based.

These all involve sharing the time spent between a General Practice post and the Specialty that is linked to that practice.

The opportunity frequently exists to continue in the practice for a further six months, as a normal GP practice post.

Community- based Medicine for the Elderly

This is an exciting new initiative of 6 months, based in a General Practice but shared with Peppermill Court, a community unit for the Elderly, and also with some ward work at York District Hospital .  

The post offers good experience in rehabilitation of elderly medical patients, acute and ongoing management of stroke patients, community-bases rehabilitation services, multi-agency and interdisciplinary assessment of need and and provision of care.  All this includes at least one and a half hours guaranteed teaching time per week.

 

Community-based Paediatric Post

This is based at the Haxby Health Centre with the time split between Practice work and Paediatrics in the York District Hospital.  The focus of work in the General Practice component of the post is on child health.  The practice has a large population of families with young children.

From the hospital perspective time is spent on the Children's Assessment Unit, which includes planned day cases as well as emergency referrals from General Practice. There is ward work, including a teaching round and newborne baby checks.  Further experience is gained in Out Patients, the Child Development Centre and in Community Paediatrics.  There is also involvement with the Child Psychiatry team.

 

Community-based Women's Health Post

This is based at the Sherburn-in-Elmet practice, with work split equally between the practice and the York District Hospital.  The hospital work includes Obstetrics, Gynaecology, Family Planning and Genito-urinary Medicine.  There is a combination of clinic work in both the General Practic and Hospital settings, with some of the experience being based in the community hospital at Selby.  There is practical work in Obstetrics on the Labour Ward, which includes some On Call work to widen the experience.  There is dedicated teaching time both in the Practice and in the Hospital.

 

Community-based Dermatology Post

The working week is split equally between a General Practice attachment and Out Patient experience with the Dermatology department.  This consists of 4 sessions in Dermatology at the York District Hospital, with a specific teaching slot.  

 

Community-based Research Post

This is another innovative post based in General Practice for 6 months, but also working at a Primary Health Care research unit at York University.  The GP Registrar gains training in research method and health economics, allowing the pursuit of a community-based research project. 

Following this six months, the GP Registrar remains at the Practice for a further six months, which allows the experience of chronic disease management in the community.  At the same time, it is an ideal opportunity to perform a longer piece of research if this is desired.

 

Community-based Palliative Care Post

This is another six month with the time split equally between General Practice and as part of a palliative care team at St Leonard's Hospice in York, which situated near to the racecourse. 

The post enables the GP Registrar to gain knowledge and experience of the many and varied aspects of terminal care, including Hospice, Day Hospice and Macmillan Nursing service.  By spending an additional six months in the same Practice, it also enables the GP Registrar to have a longer experience of chronic disease management in General Practice.

 

Community-based Psychiatry Post

This is a new post, which offers 5 sessions in General Practice with Dr Geddes, a Trainer who has a Clinical Assistantship in Mental Health, and 5 sessions doing Psychiatry at Bootham Park Hospital.  The GP Registrar has the choice, before commencing the post, of focussing the six months on one of four different areas -
